Project Description

TBI is defined as: traumatic brain injury, subarachnoid haemorrhage, hypoxic brain injury, haemorrhagic stroke, meningitis, encephalitis, and toxic conditions affecting the brain. TBI can significantly affect many cognitive, physical, behavioural, and psychological skills – with defining levels described as mild, moderate or severe – with some patients requiring long-term care following their injury. The scope of definition is wide, bringing with it a complex and often detached provision of services including:-

  • Surgical
  • Medical
  • Acute
  • Mental health
  • Substance Misuse
  • Community
  • Private Providers
  • 3rd Sector
  • Social care

In addition, commissioning of provision for TBI is a complex system through a series of arrangements, wherein as a result, services often function in isolation with no clear defined pathways and a lack of collaborative working. As local commissioning takes on new reforms, the procurement and planning of services faces new challenges with further cuts predicted and early suggestions that certain National Health Services in the future may no longer be provided ‘free at the point of contact’ with little indication at this stage of which services may be affected.
Amidst the reformation, JMB Health Consultancy undertook a scoping exercise on behalf of a large North West Mental Health Trust to:-

  • Understand the scope and demand for Dual Diagnosis (Traumatic Brain Injury complicated by mental illness and complex challenging behaviours)
  • The needs of those with Alcohol Related Brain Damage (ARBD) in terms of pathways establishing the need for ‘wrap around services’ that complement new RAID (Rapid Assessment Interface Discharge) models of care.
  • A Strengths Weaknesses Opportunities and Threats (SWOT) analysis approach to developments including individual qualitative overviews that introduce considerations for the process of next steps in service development.
  • An appraisal of options for service development opportunities pursued within 3 key considerations: TBI rehabilitation for Dual Diagnosis; step-down from Secure Services; and wrap around support models for Alcohol Related Brain Damage (ARBD).
