The potential for recovery from mental ill-health is often referred to as ‘a personal journey’ back to positive health and well-being. The new Care Programme Approach (Department of Health 2008), intends to underpin this ‘journey planning’. However, without integrated care pathways, recovery can become a restricted destination.

  • address the changing demand for care arising from an ageing population
  • offer care that is person-centred, recognising that health and social care outcomes are interdependent
  • facilitate the social integration of society’s more vulnerable groups through better access to flexible community services
  • improve system efficiency through improved care coordination
