Project Description

In support of the Dementia QIPP and in line with recommendations from the Dementia Strategy, local commissioning from a North West CCG had identified investment for the implementation of ‘Care Navigators’ who would advocate for people with a newly confirmed diagnosis of dementia and their carers and help people to navigate their way through a very difficult and challenging period in their lives. This extremely valuable and newly appointed care navigation team also supported people and their families and carers with concerns and support informed decision making around gaining diagnosis status, both pre and post diagnosis.

JMB Health Consultancy was delighted to have worked with staff, service users and their families to:

  • Evaluate activity (both planned and actual) against original targets
  • Explore in depth how the role had evolved and its effectiveness in supporting those with dementia
  • Make recommendations for the future of the role within new models of care
  • Secure investment for the future of service provision
  • Utilise findings for service expansion elsewhere within neighbouring CCG footprints
